Annual Overnight Residential Parking Permit Requirements
• Proof of residence is required to obtain the permit.
• Full payment ($10) is necessary to receive the Parking Sticker or a Temporary Permit.
• A Temporary Permit is valid for 30 days; proof of residence must be uploaded or sent before it expires. Mail applications do not include a temporary permit.
• The permit application, renewal, or replacement requires a non-refundable fee of $10.

Required Documents (must be uploaded or mailed):
1. Valid Driver's License (address must match the application)
2. Current Vehicle Registration (address must match the application)
   If the address does not match the Maywood address, please also submit:
3. Current utility bill (gas or electric) in your name showing recent service at the address.

Utility Bill Details:
• Needed if the driver's license or registration does not reflect your current address.
• Must match the address listed on the permit application.
• At least two documents showing the same address are required; all must be legible.
• Submitted documents will not be returned.

Steps to Obtain the Permit:
1) Complete the online form or download the mail-in application.
2) Submit proof of residency in one of the following ways:
   •Upload online or mail to: City of Maywood, c/o Parking Permit
      Fulfillment Center P.O. Box 11370 Santa Ana, CA 92711.
3) Click on 'Pay by credit card' to purchase online.
4) Once verified and paid, the parking permit sticker will be mailed to you.
5) Each permit requires a separate application.

Important:
• Applications are pending until all steps are completed.
• Failure to complete the process before the temporary permit expires may result in citations.
• You must have access to a printer to print your Temporary Parking Permit.
• If you are a visitor, you may go into City Hall to obtain a temporary parking permit. City Hall is open Monday through Friday, 8:00 a.m. to 5:00 p.m.

Note: Two Verifications must be submitted and must match the address on the permit application. If two verifications are not submitted, the application will not be fulfilled.
